Consent Form

The Vaccination verification requirement may be completed one of these two ways: (1) You may upload a copy of your vaccination record by accessing our SmartVault® portal here. Once your vaccination record has been uploaded to the portal, it can typically take up to 30 minutes for your status to be verified. We encourage all attendees to complete this process by March 1, 2022 to avoid delays. For your safety, and the safety of other attendees, we ask that your last booster shot be completed within a minimum of 10 days of attending the Summit.  (2) You may also choose to sign a consent form to allow us to look up your vaccination status through the California State Immunization Database (CAIR2). Pre-Registration for onsite testing is simple.  You will text “LWD2022” to (917) 456-9396.  You will then receive a message with a link to register for your rapid antigen test and sign the consent form. After a quick one-time registration, you’ll follow instructions to comply with health requirements and reserve your daily onsite rapid test. Registration will be open beginning March 1, 2022 through the duration of the Summit.  Please complete your registration before arrival.  When you arrive at the Summit, check in first at the welcome table.  Then, follow signs to the Testing Lounge located in the Monarch Wing of the hotel.

 

If you receive a positive test result prior to travel, please email our CHP Health Team at covidmd@sharedharvestfund.org to discuss next steps and possible quarantine guidelines.
